Case management begins with a comprehensive in-home assessment to determine a senior’s preferences and needs. Our care managers work with seniors and family members to create a plan specifically tailored to the senior’s situation.
Case Management:
- Provided by P. Harrison & Associates experienced case managers (social workers and nurses).
- Assessments consider the senior’s strengths and preferences as well as the challenges and risks in the senior’s situation. Our care managers evaluate
- What is important to the senior’s quality of life
- Physical health
- Memory and mood
- Ability to handle day-to-day needs
- Who is willing and able to help the senior
- Legal and financial issues
- Our case managers help identify resources and assist with putting services into place.
- Our case managers are experienced in dealing with the challenges of aging including:
- Seniors who need assistance but are reluctant to accept help.
- Caregiver stress and burnout
- Memory loss, including Alzheimer’s disease and other dementias
- Chronic illness
- Challenges with clutter including hoarding
- Navigating benefits and subsidized services

For a one or two visit in-home consultation and assessment, we can make home visits anywhere in the Dallas Metropolitan area.
What are the charges for case management
Care management is offered on a sliding-scale based on the senior’s income.
